his article describes two clinical cases of children with midbrain tumors who underwent surgery at the N.N. Burdenko National Medical Research Center of Neurosurgery. The efficacy of preoperative magnetic resonance (MR) tractography and intraoperative neurophysiological monitoring is examined. These modern techniques aid in determining the optimal surgical approach, extent of resection, and preservation of the patient’s functional status. Conclusion. Reconstruction of the corticospinal tract using MR tractography with subsequent intraoperative monitoring are effective methods in the surgical treatment of midbrain tumours.
