Management of adolescent patients with maxillary micrognathia requires complex orthodontic and surgical treatment. However, the surgical stage of treatment is a step that, according to the authors, requires certain optimization. For this purpose, the authors developed a compression-distraction device for distraction of the maxilla in the sagittal plane. The design of this device allows the parallel use of a palatal compression-distraction device for simultaneous distraction of the maxilla in the transverse plane. This allows achieving the planned sagittal and transverse sizes of the maxilla in adolescents without the use of uncomfortable face masks or the need to wait for maturity to perform conventional orthognathic surgery. This clinical case study demonstrates the technique of bidirectional simultaneous maxillary distraction in two planes in the adolescent patient.
